Job summary

Pharmacy Careers is seeking MTM Pharmacists to join our Medication Therapy Management team. Conduct comprehensive reviews, coach patients on adherence, and optimize outcomes, often from home or with flexible hybrid schedules.

You will collaborate with prescribers, educate patients, ensure CMS MTM compliance, and help reduce hospitalizations while enjoying meaningful clinical work and competitive pay and benefits.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Licensed pharmacists encouraged, including new graduates.
• Ambulatory, hospital, or community MTM experience is a plus.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
