UnitedHealth Group, Mountlake Terrace Assured Home Health, part of LHC Group, is seeking a Social Worker - Per Diem. The role delivers medical social services under physician direction and an interdisciplinary team to address social and emotional factors affecting health status.

Pay ranges from $36.70 to $55.05 per hour; benefits include a comprehensive package and incentive programs. WA-specific credentials may be required if not licensed as a social worker; CPR and licensure in the state are

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• WA: If not licensed as a social worker, Agency Affiliated Counselor credential is required.
• 1+ years of social work experience in a health care setting
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence

Responsibilities

• Provide medical social services under physician direction and interdisciplinary group to assist in understanding social and emotional factors related to health status and development of coping mechanisms.
• Collaborate with the team to support patients and families and contribute to care planning.

As the Social Worker - Per Diem, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

Required Qualifications:

• Master's Degree from a school of Social Work accredited by the Council of Social Work

Additional State Specific Requirements:

• WA: If not licensed as a social worker, Agency Affiliated Counselor credential is required.
