Responsibilities

• Works as a multi-disciplinary team member.
• Assesses and treats patients/clients/families with emotional/behavioral/health problems.
• Provides social work services to patients/clients/families with behavioral, social, financial/economic, and environmental problems.
• Interviews patients/clients and their families.
• Evaluates underlying social and emotional factors; identifies needs, goals, and treatment preferences; coordinates services; facilitates case management.
• Provides clinical counseling and individual therapy to help positive behavior changes.
• Assists patients in treatment planning.
• Provides direction to clinical team for implementing treatment plans.
• Provides crisis intervention services to patients and families.
• Advises family caregivers; provides case and care management to promote health.
• Provides information and counseling; makes referrals for other services.
• Maintains contact with patients to adjust needs through facilities or outside services.
• May execute discharge planning responsibilities.
• Participates in social and health programs; may assist in planning events.
• May travel to patients' homes to render services.
• Demonstrates an understanding of diversity and I CARE values.
• Enhances a safe environment for colleagues, visitors, and patients.
