Angels Care Home Health in Broken Bow, OK is seeking a Part-time Social Worker (MSW) to join our team. The MSW provides coordinated home health care, performs psychosocial assessments, counseling, and referrals to meet patient and family needs.

Requirements include a Master’s Degree in Social Work, state certification, a current driver’s license, at least one year MSW experience in health care, and knowledge of community resources.

Qualifications

• Master’s Degree in Social Work.
• State Certification.
• Current State Driver’s License.
• One year of experience as an MSW in a health care agency.
• Working knowledge of community resources.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Assess home health patient to identify psychosocial, financial, environmental, and community needs.
• Coordinate care and referrals to meet patient/family needs.
• Maintain documentation and participate in case conferences and education requirements.
• Promote agency philosophy and policies to ensure quality of care.
