Job summary

FOX Rehabilitation is expanding in Putnam Valley, NY, and we’re seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes or senior living communities. Our Geriatric House Calls model supports autonomy, flexible scheduling, and meaningful patient outcomes, with telehealth options to extend access.

You will perform evaluations, develop plans of care, deliver evidence-based therapy, document in FOX’s EMR, and collaborate with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and quality of life
• Document in FOX EMR according to regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote aging in place
• Provide telehealth/virtual care when appropriate
