Prism-Health seeks a licensed Nurse Practitioner to support mobile school and community health events across Decatur and surrounding areas. You will perform school and sports physicals, administer vaccines, and deliver preventive care in fast-paced, team-based settings.

Qualifications

• Active Illinois Nurse Practitioner license and BLS certification required.
• Experience conducting school and pediatric physical exams preferred.
• Experience administering pediatric vaccines and using EMR systems.

Responsibilities

• Conduct school-required physical examinations for children and adolescents.
• Perform vaccinations and immunizations at events.
• Provide preventive health screenings and collect specimens as needed.
• Lead staff onboarding and provide supervision as needed.
• Document services in the EMR and maintain quality care.
