Job summary

Hero Health Practitioners, LLC is seeking a compassionate Nurse Practitioner to provide comprehensive primary care in skilled nursing, long-term care, and assisted living communities across Missouri.

The NP will join a mobile primary care team, collaborating with residents, families, nursing staff, therapists, and physicians to improve outcomes and reduce hospitalizations. Applicants must hold NP, Missouri licenses, and CCM/UHC-friendly credentials.

Qualifications

• Master's degree or higher in NP program.
• National board certification (FNP, AGNP, AGPCNP, or GNP).
• Active Missouri APRN and RN licenses.
• Eligible to practice under a Missouri Collaborative Practice Arrangement.
• DEA and Missouri BNDD registration (or ability to obtain).
• Medicare (PECOS) and Medicaid enrollment eligibility.
• Current BLS certification.
• Valid driver's license and reliable transportation.
• Experience using Electronic Health Records (EHR).

Responsibilities

• Provide comprehensive primary care to residents in skilled nursing, long-term care, and assisted living settings.
• Serve as the primary on-site NP for the assigned facility, collaborating with residents, families, nursing staff, therapists, and physicians.
