Job summary

Riverside Community Care is seeking an Integrated Team Counselor - Mobile in Massachusetts to provide client-centered interventions within Adult Community Clinical Services. You will participate in assessments, care planning, and community-based rehabilitation, helping individuals develop life skills and access supports to live safely in the community.

Responsibilities include education, coordination with services, and maintaining clinical records, including MAP documentation.

Responsibilities

• Provide input to person-centered assessment, service planning and advocacy; coordinate with other services and the community.
• Deliver rehabilitation interventions and skills training in community environments.
• Educate and train clients in life skills including budgeting, banking, transportation, housekeeping, personal hygiene, self-medication, nutrition and health.
• Model and teach skills for positive relationships and conflict negotiation.
• Assist clients with accessing necessary and appropriate health care.
• Maintain regular clinical records and document medication administration in line with MAP policies.
