The Mobile Integrated Health Care (MIH) Provider will enhance the healthcare continuum through targeted in‑home care and care coordination. The MIH model expands the traditional role of EMS by shifting from a strictly emergency‑response framework to a proactive, preventive, and patient‑centered approach. The MIH provider operates as a mobile clinical resource that bridges gaps between EMS, primary care, hospitals, and social service agencies—ultimately improving outcomes for high‑risk and vulnerable populations, decreasing ED visits and readmissions and decreasing over‑utilization of the 911 system.
