Job summary

Qualifications

• MSW degree required.
• Licensed Master Social Worker in NY State required.

Responsibilities

• Screening telephone referrals.
• Psychosocial evaluation and assessment of mental health service needs.
• Develops and implements short-term service plans.
• Provides counseling and care management and referrals.
• Prepares case histories and records.
• Engages resistant clients to accept mental health services.
• Participates in interdisciplinary team meetings and case conferences.
• Coordinates linkages with other service agencies and providers.
• Provides outreach services to community members in need of mental health intervention.
• Mentors or supervises junior staff as needed.
• Assists in program coordination and policy development.
