Job summary

The University of Rochester is seeking a Crisis Specialist to join the Monroe County Mobile Crisis Team. You will respond to mental health crises, interview patients and families with a Crisis Therapist, and provide emotional support and resource linkage across various settings.

You will facilitate referrals, work with community agencies, and assist with ensuring access to Medicaid and social services while promoting culturally informed care. This role may include after-hours responses.

Qualifications

• Associate's degree required.
• 2 years experience in human services, preferably in a psychiatric crisis or emergency setting.
• Ability to speak Spanish, Vietnamese or be fluent in American Sign Language preferred.

Responsibilities

• Responds to patients in mental health crisis at off-site locations.
• Interviews patients and families in concert with Crisis Therapist.
• Provides patients and family members emotional and tangible support, advocacy and linkage to resources.
• Responsible for facilitating the referral process.
• Confers with other community agencies, Therapists, and law enforcement agencies as necessary at the request of the Crisis Therapist.
• Enhances patient efforts in receiving Medicaid and other social services.
• Provides support and education to families and agencies involved with the patient in psychiatric crisis.
• Promotes and ensures continued staff education around multicultural factors which affect care and treatment of patients.
• Visits patients in their community setting to assess progress toward meeting the goals of the care plan.
• Teaches, guides, and assists patients and family members to ensure their clear understanding of the patient's needs and expectations of the specific care plan.
• Makes recommendations to mental health professionals regarding treatment plans.
• Maintains contact with psychiatric service providers to assess placement appropriateness and progress.
• Documents patient contacts and progress in the medical record and assists patients with paperwork necessary to ensure linkage to resources.
• Participates in on-call group as back-up to Life Line during evening/weekend hours.
• Responds after hours to psychiatric crises in the community and attends weekly meetings.
