Gentiva's Heartland Home Health in Midland, MI is seeking a Registered Nurse (RN) Home Health Admissions with a Monday-Friday schedule to welcome patients into our program and ensure a smooth admission experience.

Qualifications

Active RN license in Michigan or eligibility to obtain.
1+ year of high acuity nursing experience.
Strong organizational and communication skills.
Valid driver’s license with reliable transportation.
Current CPR certification.

Responsibilities

Conduct comprehensive initial assessments to determine patient eligibility and care needs.
Initiate the Plan of Care per physician orders and standards.
Document assessments and admission records accurately and promptly.
Serve as liaison between referral sources, patients, families and the team.
Maintain compliance with federal, state and agency policies.
Uphold professional nursing standards while delivering compassionate care.
