Job summary

Metropolitan Family Services in Chicago, IL is hiring a full-time Mental Health Juvenile Justice/Future Leaders Youth Engagement (MHJJ/FLYE) Case Manager to support youth and families on the Southeast Team. You'll assess needs, develop action plans, coordinate wraparound services, and advocate for clients.

  • 1+ year of case management experience in a social service or healthcare setting, preferred.

Responsibilities

  • Conducts thorough assessment of referred and accepted clients and evaluates needs.
  • Develops a written action plan with clients to meet goals.
  • Provides wraparound services, referrals, and linkages to support client needs.
  • Advocates, provides case management and mentoring to individuals and families.
  • Collaborates with community resources and monitors progress toward goals.
  • Adheres to contract, grant, and program requirements.
  • Engages clients in settings to practice healthy behaviors and coping skills.
  • Reassesses status and updates action plans within required timeframes.
  • Collaborates with staff in planning and implementation of action plans.
  • Handles emergency and crisis situations with supervisory approval.
  • May authorize limited financial assistance to clients.
  • Other duties as assigned.

We are now hiring for a full-time Mental Health Juvenile Justice/Future Leaders Youth Engagement (MHJJ/FLYE) Case Manager to join our Southeast Team!

SALARY: The average salary for this position is in the $45,000- $49,000 range. Compensation is commensurate with years of related experience, position requirements, and candidate qualifications.

ESSENTIAL JOB FUNCTIONS
  • Conducts thorough assessment of referred and accepted clients. Evaluates problem and service needs of clients with client input.
  • Develops a written action plan with clients to meet clients' desires and needs.
  • Works with client to supplement service needs with appropriate wraparound services, referrals and linkages.
  • Provides, as appropriate, supportive help, advocacy, case management and mentoring to individuals and to families in coping with environment and interpersonal relationship issues.
  • Works collaboratively with community resources as well as develops a knowledge of resources and helps clients to utilize these resources. This includes referrals and linkages to community resources for successful graduation/discharge from the program.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Engages client in settings that may present a challenge for the client for the purpose of learning/practicing/developing healthy behaviors and coping skills.
  • Within appropriate and required time frames, reassesses and monitors client status and the adequacy of the action plan.
  • Collaborates and participates with other program staff in the planning and implementation of action plans.
  • Subsequent to consultation with supervisor, handles emergency and crisis situations within target population.
  • With supervisory approval, authorizes limited amounts of financial assistance to own clients.
  • Other duties as assigned.
QUALIFICATIONS
  • Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university, required.
  • 1+ year of case management experience in a social service or healthcare setting, preferred.
KNOWLEDGE, SKILLS, and ABILITIES
  • Strong interpersonal and time management skills.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
ADDITIONAL REQUIREMENTS
  • Satisfactorily pass all relevant screenings and criminal background checks.
TRAVEL/TRANSPORTATION REQUIREMENTS
  • Driving for work required with transportation of clients preferred in a personally owned vehicle. Travel between sites not required.

While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
