Job summary

The Child Center of NY is looking for a dedicated case manager to support children and families across NYC. You will coordinate administrative, clinical and evaluative activities to help clients meet treatment goals and participate in multi-disciplinary meetings to reassess progress.

Responsibilities include documenting care in line with regulatory standards, providing educational counseling, and conducting home or school visits as needed.

Qualifications

  • Bachelor’s degree in social work, psychology or counseling.
  • 2 years’ experience in a community-based mental health or case management setting.
  • Bilingual may be required depending on client needs.
  • Good interpersonal skills and ability to work with diverse groups.

Essential Functions:
  • Performs case management, including bringing together administrative, clinical and evaluative activities, treatment services, community agencies and other resources needed to assist clients to meet the goals of their treatment plan. Supports and advocates for clients in multi-disciplinary meetings, and periodically reassess and evaluates what has been accomplished.
  • Maintains appropriate professional client care documentation including case management and individual notes in compliance with all city, state and federal standards of practice. Provides educational counseling and advocacy.
  • Conducts home, school and other visits as needed.
  • Meet performance expectations/metrics, as defined for the position within the assigned program.
  • Performs other related duties as assigned.
Minimum Qualifications:
  • Bachelor’s Degree in Social Work, Psychology or Counseling.
  • 2 years’ experience working in a community-based mental health or case management setting
  • Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali and others, may be required depending on client needs.
  • Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.
  • Proficiency in Microsoft Office.
  • Ability to plan and carry out assignments independently.
  • Ability to prioritize, adhere to timelines and multi-task.
Qualifications Preferred:
  • Masters degree in mental health relevant to working with children, adolescents and/or young adults and their families.

  • Safe and successful performance, including meeting productivity standards.
  • Maintain an understanding of the principles, methods and processes needed to perform the job. Attends staff meetings, seminars and in-service training as assigned.
  • Demonstrates the ability to complete work in an acceptable manner by the time and/or date established with accuracy, neatness and thoroughness and minimal amount of errors.
  • Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times.
