MidMichigan Health in Michigan seeks a full-time therapist and case manager for the Psychiatric Partial Hospitalization Program and Case Management, collaborating across departments to deliver therapy and coordinate patient care. Duties include program development, documentation, and family education.

Requirements include a Master's degree in psychology, social work or professional counseling, Michigan licensure within 3 years if licensed temporarily, BLS, fingerprinting, and team advanced

Competenze

• Master's degree in psychology, social work or professional counseling.
• Licensed by Michigan: MSW, Psychology, or Professional Counselor with full licensure within 3 years if currently limited.
• Minimum 1 year mental health service delivery in past 5 years.
• BLS certification required.
• Fingerprinting (FINGERPRIN) required.
• TEAM ADVANCED certification within 6 months (24 months for hires before 2023).

Mansioni

• Assessment, development, implementation, evaluation and documentation of patient treatment programs.
• Assess patients at admission and collect data to determine status.
• Improve patients' physical health and maintain safety.
• Screen applicants and make recommendations for treatment.
• Maintain program patient medical records.
• Provide group or individual therapy.
• Documentation of Bio Psycho Social Assessments and Treatment Planning.
• Respond to patient conditions using critical thinking.
• Participate in admission process and liaison program data collection.
