Job summary

Concordia-IRMC VNA is seeking a full-time Mental Health Registered Nurse RN to deliver collaborative psychiatric and medical nursing care to homebound patients in Indiana. The service aims to prevent hospitalizations and support community living and quality of life.

Typical hours are Monday–Friday 8am–5pm with some weekends, evenings, and holidays. The role offers a $5,000 retention bonus, premium pay rates, tuition assistance, strong benefits, and a 1:1 patient ratio.

Qualifications

• Licensed RN with Pennsylvania license or eligible to practice in PA.
• Master’s degree in psychiatric/mental health nursing preferred or required.

Responsibilities

• Perform initial assessment visits using observation and assessment skills.
• Provide psycho-educational interventions for patient and family.
• Administer medications and provide teaching on medications and coping skills.
• Coordinate post-hospitalization follow-up to support transition to home care.
• Educate patients and families about disease processes, safety, and self-management.

QUALIFICATIONS for the Mental Health Registered Nurse RN position:

• Registered professional nurse with a current valid Pennsylvania license. and has special training and/or experience beyond the standard curriculum required for a registered nurse such as:
  • Registered Nurse with Master’s degree in psychiatric or mental health nursing.

OR

• Registered Nurse with Two years of recent nursing experience within the last 3 years in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or another outpatient psychiatric service
• Reliable, independent means of transportation to make home visits is required, until a company car becomes available. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

DUTIES AND RESPONSIBILITIES for the Mental Health Registered Nurse RN position:

• Makes initial assessment visit utilizing observation and assessment skills
• Utilizes psycho-educational approaches with the patient and family directed toward increasing the patient’s level of knowledge and control over his/her illness, and enhancing coping skills through: Symptoms and Illness management, Instructs self-care, mental and physical well-being, promotes independence and patient’s rights.
• Administers medications, Medication teaching, Coping Skills, and Communication Skills
• Provides post hospitalization follow-up to assist in bridging the patient’s transition from hospital to community. Provides therapeutic interventions and psychotherapy interventions
• Provides psycho education such as teaching/training with disease process, symptom and safety management, coping skills and problem solving
