Job summary

Dominion Hospital in Falls Church, VA is seeking a Mental Health Professional I - Activity Therapy on a PRN basis. You will lead psycho-educational groups and play a key role in discharge planning and safety coordination with the care team.

The role requires a Bachelor's in Social Work or related field, BLS for Healthcare Providers, and Crisis Prevention Institute training. Experience in mental health is preferred; a supportive, family-like workplace awaits.

Dominion Hospital, the only freestanding mental health facility in Northern VA, is seeking an experienced Mental Health Professional I - Activity Therapy to support our Activity Therapy program. In this key role, you will be responsible for high quality, patient-centered care by developing discharge plans that address the psychological, social, cultural and/or financial needs that impact the patient's care and recovery. The Mental Health Professional I - Activity Therapy serves as a liaison between the clinical team, patients, families and next level of care providers in all aspects of patient care, in accordance with the scope of practice.

What You’ll Do
  • Conducts individual, group and family psycho-educational and process groups.
  • Contributes to treatment plan development and attends treatment planning meetings and reviews on an ongoing basis.
  • Assess environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
  • Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
  • Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
  • Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge.
  • Conducts post-discharge follow-up as indicated.
  • Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.
  • Develops and maintains a community resource guide including access to public assistance/alternatives.
  • Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.
  • Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities/resources.
  • Identifies cultural considerations, gender identity and preferred language for patients and family.
  • Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
  • Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers.
  • Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.
  • Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.
  • Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.
  • Demonstrates knowledge and role in hospital codes.
  • Assists with Process and Quality Improvement initiatives.
  • Performs other duties as assigned.
Qualifications

Education: Bachelor’s Degree in Social Work or related field-Required

Certifications: Basic Life Support for Healthcare Providers required and Crisis Prevention Institute required as a part of the general hospital orientation.

Experience: Experience in mental health or related field-Preferred
