Job summary

Merakey is seeking a Mental Health Professional to join our ACT Team in Shelocta, PA, supporting Indiana and Armstrong County. You will deliver recovery‑oriented services to adults with severe mental illness in home and community settings.

Responsibilities include assessing and developing individualized treatment plans, coordinating therapy and medication management, collaborating with psychiatrists, nurses, and other professionals, and conducting visits using a personal vehicle for

Qualifications

  • Master’s degree in counseling, social work, psychology, or a related human service field.
  • 2 years of clinical experience working with adults with severe and persistent mental illness preferred.
  • A valid driver’s license is required. Travel is necessary to provide support across Indiana and Armstrong County.
  • Knowledge and understanding of recovery principles and recovery‑driven systems.

Position Details

As a member of the Assertive Community Treatment (ACT) Team, the Mental Health Professional plays a vital role in delivering comprehensive services to individuals with severe and persistent mental illness.

  • Assess, plan, and develop individualized treatment plans for individuals with severe and persistent mental illness.
  • Coordinate and provide therapy, medication management, and resource linkage services.
  • Collaborate with a multidisciplinary team including psychiatrists, nurses, substance abuse professionals, employment specialists, housing specialists, and peer specialists.

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have benefited from traditional outpatient programs. We offer tailored services emphasizing relationship‑building and active involvement in enhancing functioning and symptom management.

Services involve weekly home visits, provided either in individuals’ residences or within the community, utilizing personal vehicles for client transportation and community‑based tasks.
