Job summary

RADIAS Health is seeking a Mental Health Practitioner to support clients in a Housing Navigation Program. You will lead housing searches, provide outreach, and help clients regain skills for independent tenancy while coordinating with the treatment team.

The role requires a Behavioral Health Bachelor’s degree or 4,000 hours of supervised experience, and a valid driver’s license. Onsite in Saint Paul with comprehensive benefits and a 40-hour work week.

  • 2,000 hours of work experience providing supportive services to vulnerable populations.
  • No college degree with 4,000 hours supervised work experience acceptable.

Responsibilities

  • Engage in completing Housing Focused Plans with referred clients.
  • Monitor progress and modify Housing Support Plans; communicate with treatment team.
  • Maintain records of progress and documentation for each contact.
  • Assist clients to create plans toward service goals in regular meetings.
  • Schedule appointments and bill for services as per policies.
  • Coordinate with providers and community resources for client care.

Starting Salary Range: $59,773-$61,860 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change

RADIAS Health is hiring a Mental Health Practitioner to join our Housing Navigation Program in providing services to individuals experiencing long term homelessness who have mental illness and/or substance use disorders. The Housing Navigation program helps clients find their own independent housing within Ramsey County and provides supportive services to help them maintain their housing.

The Mental Health Practitioner leads the housing search and provides support and advocacy to overcome barriers affecting housing in the past. In this role, you will provide outreach, housing transition, and tenancy sustaining services to vulnerable adults. The primary function of the practitioner is to assist the individuals served in restoring skills essential for managing their mental illness, treatment, in order to support ongoing tenancy in independent.

Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week

This position works onsite at the RADIAS Health downtown Saint Paul office

Duties and Responsibilities:
  • Provide instruction and monitoring on the identified service areas indicated on the client’s Housing Focused Person Centered Plan, other assessments, and other recommendations by client’s treatment team.
  • Engage in completing some Housing Focused Person Centered Plans with people referred.
  • Continually monitor progress and modify the Housing Support Plan and Housing Support Crisis Plan as appropriate. Communicates changes in service needs and recommendations to Clinical Supervisor/Mental Health Professional, Case Manager, and rest of treatment team on an ongoing basis to coordinate care.
  • Maintain records of progress for each face-to-face meeting and telephone contact with client. Documentation will include an assessment of client’s ability to function and manage in their own apartment.
  • Assist clients to create a plan to work towards service goals in regular meetings with the client.
  • Schedule appointments to see the person to assist in requested services for the amount of time and frequency agreed upon in the person's support plan.
  • Bill for services by completing contemporaneous documentation as required by program policies and procedures.
  • Intervene as necessary for emergencies and/or when he/she assesses that the person may need when in crisis. Connect the person to appropriate emergency services, doctors, staff, and emergency family contacts when necessary.
  • Coordinate and cooperate with other agency and community providers to provide the best possible care for the individual, including but not limited to landlords and other community resources. Practitioner will complete all necessary Release of Information and documents for communication with other providers and keep these current for each person served.
  • Maintain records of contact with other providers regarding care needs.
  • Assist person served in obtaining and maintaining benefits including housing subsidies and other economic assistance, as well as providing assistance in budgeting monthly funds to maintain housing.
  • Must be able to transport clients in the community.
  • Candidate will have a Bachelor's degree in behavioral health sciences plus 2,000 hours work experience providing proving supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
