Job summary

Mahajan Therapeutics is seeking a hard-working Mental Health Case Manager to join our outpatient clinic team. The role supports therapists and families, coordinates ISP services, and helps clients access community resources with attention to detail and adherence to policy.

The Case Manager will liaise among therapists, referral sources, and families; participate in treatment staffing; assist with crisis management and documentation; and contribute to quality assurance efforts under supervision

Qualifications

• 1 year experience working with adolescents, teens, and adults in mental health settings.
• Proven ability to work professionally with colleagues, physicians, providers, patients, and families to promote teamwork.
• Competent with MS Office applications (Word, Excel, PowerPoint).
• Adaptable in a challenging, dynamic environment within a matrixed organization.
• Satisfactory references and background/drug screen.

Responsibilities

• Serve as liaison between therapist, referring agency, and family; arrange visits and coordinate transportation.
• Ensure ongoing communication among family, referral sources, and Mahajan Therapeutics.
• Attend court hearings as appropriate and ensure documentation follows policy.
• Prepare reports for treatment staffing and represent client responses to treatment.
• Facilitate CSPT groups and participate in quality assurance activities.
• Assist in in-service training and supervise sessions with prepared agendas.
• Monitor ongoing client needs and support independence in daily living skills.
• Coordinate ISP services and link clients to natural supports and formal community services.
• Support crisis management and stabilization efforts and advocate for clients.
• Educate and train clients based on individual needs and readiness to learn.

Full Time - Mental Health Case Manager

Mahajan Therapeutics is looking for hard-working and conscientious Case Manager to perform various administrative tasks with a keen eye for detail and works under the supervision of the Case Manager Supervisor and Clinical Director. The Case Manager’s primary job function is to enhance the overall treatment process by assisting therapists to meet the overall treatment needs of clients. Performs other duties as assigned.

Responsibilities:

• Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients’ needs are met.
• Ensure communication between family, referral source and Mahajan therapeutics is agreeable and ongoing.
• Attend court hearings as deemed appropriate.
• Ensure documentation occurs according to policy.
• Attend treatment staffing with prepared report that accurately represents client’s response to treatment.
• Facilitate individual and group CSPT as assigned.
• Function on agency quality assurance or other committees as assigned.
• Assist with in-service training as directed by Clinical Director or CM Supervisor.
• Attend supervisory sessions with prepared agenda.
• Ongoing assessment of client needs
• Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian
• Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian
• Coordination of the ISP, including:
  • Services identified in the ISP
  • Assistance with accessing natural support systems in the community
  • Linkages to formal community services and systems
• System monitoring
• Coordination and assistance in crisis management and stabilization
• Advocacy and outreach
• As appropriate to the care of clients, education and training specific to the individual needs, abilities and readiness to learn
• Mental Health interventions that address symptoms, behaviors, thought processes, etc., that assist a client in eliminating barriers to seeking or maintaining education and employment
• Activities that increase the client’s capacity to positively impact the client’s environment
• Adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
• Assume additional duties as assigned by Clinical Director or CM Supervisor.

Qualifications:

• 1 year e xperience working with adolescents, teens, and adults, emotionally disturbed population, mental health experience
• in a pleasant and professional manner with other employees, physicians, providers, patients, and patient’s families to promote a team effort through cooperation and respect for fellow employees.
• Proven competency with MS Office applications, including Word, Excel and PowerPoint required.
• Adaptability in a challenging and dynamic environment; understand and work well in a matrixed organizational structure required.
• Satisfactory references from employers and/or professional peers and satisfactory criminal background check.
• Satisfactory drug screen
