Job summary

Cogir Senior Living in Scottsdale, AZ, seeks a Memory Care Resident Care Coordinator to bridge residents, families, and the care team, ensuring a safe, supportive living environment. You’ll assist in scheduling, supervising staff, and coordinating with external providers to deliver quality assisted living services.

The role includes medication administration support, coordinating admissions, and guiding care planning with on-site teams while adhering to state regulations and company policies.

Qualifications

  • High School Diploma or equivalent.
  • Valid state medication administration license or meet state requirements.
  • Current First Aid and CPR license or ability to obtain.
  • 2+ years of medication administration and care experience.

Responsibilities

  • Schedule and assign caregiving and med tech shifts to ensure coverage and compliance.
  • Participate in recruitment and interview process for new care team members.
  • Supervise, train, and support care team, promoting teamwork and adherence to standards.
  • Supervise the medication room and orders with med techs and caregivers.
  • Maintain accurate resident records, including care plans and medical documentation.
  • Coordinate care planning with home health agencies on-site and with policies.
  • Coordinate admissions with the sales team and supervise move-ins.
  • Complete family/resident admission interviews.
  • Facilitate communication between residents, families, and providers for seamless care.
  • Address residents’ special needs, requests, and complaints.
  • Assist with medication administration and direct care when necessary.
  • Understand community care regulations and state-specific rules.

Position Summary

Our Memory Care Resident Care Coordinator is a liaison between residents, their families, and the care team to promote a supportive and safe living environment. They support the Health and Wellness Director by assisting in scheduling, supervising staff, and ensuring that systems are in place to provide quality assisted living services in all resident care areas. Duties include problem-solving resident concerns and coordinating care with the team and external providers. Additionally, they assist with medication administration and direct resident care when necessary.

Key Responsibilities
  • Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Participate in the recruitment and interview process for new care team members.
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
  • Supervise the medication room and orders, working with med techs and caregivers.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with home health agencies on-site, working within community and company policies.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Complete family/resident admission interviews.
  • Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
  • Address residents’ special needs, requests, and complaints.
  • Assist with medication administration and direct care to residents when necessary.
  • Understand the community's care regulations along with state-specific regulations.
Requirements
CANDIDATE QUALIFICATIONS
Education And Certifications
  • High School Diploma or equivalent.
  • Must have a valid state medication administration license (such as CNA/CMA/CMT/QMAP) or meet the state requirements for medication administration in assisted living.
  • Current First Aid and CPR license or ability to obtain.
Experience, Competencies, And Skills
  • At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required.
  • At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred.
  • Experience with memory care.
  • Excellent attention to detail and high integrity.
  • Excellent communication skills and teamwork mindset.
  • Strong leadership and team management skills.
  • Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
  • Willingness to participate in weekend rotation.
