Job summary

Legacy House of Southern Hills in Henderson, NV is seeking a full-time Memory Care Program Coordinator to lead our memory care unit. You will conduct care assessments, develop individualized plans, supervise staff, and partner with families and medical professionals to ensure compassionate, compliant resident care.

We offer a $50,000 annual salary, 401(k) with company match, comprehensive insurance, an HSA, PTO, and generous referral and discount programs.

Qualifications

• Care Assessment & Planning: Conduct initial and ongoing evaluations of residents' cognitive and physical abilities and develop individualized care plans in collaboration with the care team.
• Team Leadership: Supervise care staff to ensure high-quality support is provided and train team members on effective memory care techniques and communication methods.
• Resident Engagement: Lead daily activities that promote cognitive health and social connection, including individual and group programs tailored to memory care needs.
• Family Support: Act as the primary contact for families, offering regular updates and providing resources to help them navigate memory-related challenges.
• Documentation & Compliance: Maintain detailed and accurate records of resident progress and care and participate in meetings to review and adjust care strategies.
• Healthcare Collaboration: Partner with medical professionals to coordinate integrated care and ensure services meet regulatory standards and memory care best practices.
• Environmental Oversight: Monitor and adjust the memory care environment to ensure it is safe, secure, and supportive of residents' well‑being and independence.

WHEN WILL YOU WORK?

This full-time position offers a consistent Tuesday through Saturday schedule.

WHAT DO YOU NEED TO BE OUR MEMORY CARE PROGRAM COORDINATOR?

• Medication Technician certification
• Experience in healthcare or social services, particularly in gerontology or memory care
• Knowledge of memory disorders and understanding of therapeutic interventions
• Ability to manage a team and promote a positive, supportive environment
• Strong communication and interpersonal skills
