Job summary

Remember Me Senior Care is seeking an LPN to promote direct resident care and a team approach with aides, while ensuring the Resident’s Plan of Care is followed. The role emphasizes dementia-specific caregiving and active involvement in resident activities.

The LPN will oversee medication delivery, documentation, and infection control, with a focus on safety, communication with families, and coordinating diets prescribed by physicians.

Qualifications

• High School diploma or GED required.
• Current Tennessee LPN license required.
• One year LPN experience in assisted living/home health preferred.
• Competence in assessment, injections, follow up and triage.

Responsibilities

• Provide direct resident care and oversee caregivers to follow the Resident’s Plan of Care.
• Maintain medication delivery system, MAR documentation, narcotic count, and timely entry.
• Promote wellness objectives, safety, infection control, and fall prevention programs.
• Communicate promptly with families; report concerns to supervisor and/or Director of Nursing.
• Coordinate with the Dining Service Director to ensure physician-prescribed diets are followed.
• Maintain documentation of wellness care and resident responses to care.
