Job summary

Trinity Springs, a Cassia community in Oxford, FL, seeks a licensed Practical Nurse (LPN) to join our memory care team as Memory Care Coordinator. This full-time role includes a weekend rotation and a 10:00 AM–6:00 PM shift with additional shifts as needed.

You will provide high-quality clinical care, administer medications per orders, coordinate resident care, and document using our charting system while collaborating with families and the Clinical Manager to address changes in condition.

Qualifications

• Must have a current Florida LPN license and be in good standing.
• Experience in Assisted Living preferred.
• Knowledge of FL assisted living AHCA regulations.
• Must have strong communication and organizational skills.
• Self-starter with a desire to be a part of a team.

Responsibilities

• Display a courteous attitude and respect for residents, families, and staff.
• Coordinate, oversee, and provide resident-related care for assisted living and memory care residents.
• Administer medications and/or treatments per physician orders and monitor effectiveness/response.
• Manage medications, supplies, and equipment including ordering, receiving, storing, and disposing according to policies.
• Document care using computerized medication administration and charting software.
• Notify Clinical Manager, attending physicians, and family members of significant changes in a resident's condition and follow up as necessary.
• Monitor nursing personnel on duty during the assigned shift.
