Job summary

Manor Lake El is seeking a Memory Care Program Manager to develop, coordinate, and implement memory care services for residents with cognitive impairment. You will supervise Memory Care Staff and work to maintain a therapeutic environment focused on residents’ well‑being.

You will lead policy development, staff scheduling, and ensure compliance with state surveys. The role involves coordinating with leadership and family members to deliver personalized care plans and ongoing training.

Qualifications

  • Bachelor’s degree in social work, psychology, gerontology, therapeutic recreation, or equivalent is desired.
  • Two years of experience in geriatric care is required.

Purpose

Develops, coordinates, and implements programs and services for residents with cognitive impairment while providing a therapeutic environment that maintains each resident’s highest level of well‑being of mind, body, and spirit.

Essential Functions
Job Functions
  • Direct and supervise Memory Care Staff
  • Develop, maintain and periodically update written care policies, procedures and manual, objectives and philosophies. Assist in developing and implementing methods for coordinating care services with other resident services.
  • Implement plans of correction for state surveys and deficiencies, complete medical forms, reports, evaluations, studies, charts, etc. Implement and maintain the continuous quality improvement program.
  • Schedule department work hours, including vacation and holiday schedules, shift and work assignments. Ensure adequate staffing and expedite work; delegate administrative authority, responsibility and accountability to resident care services supervisory associates as deemed necessary to perform assigned duties.
  • Coordinates the implementation of the Memory Care Program mission, goals and philosophy.
  • Provides or secures training to associates in the Community including training caring for residents with memory impairment and other advanced training.
  • Provides or secures the additional training/practicum experience to Resident Caregiver associates of Memory Care Program.
  • Coordinates with Resident Care Director in establishing and maintaining a staffing pattern to meet our customized care commitment.
  • Maintains a mental, physically and spiritually stimulating activity program that is geared to the individual’s preferences and capabilities.
  • Participates with the Resident Care Director in the hiring, scheduling and supervising of Resident Care Associates assigned to the Memory Care program.
  • Acts as liaison to facilitate communications among Memory Care, the community, family members and the local community.
  • Maintains as environment in the residents’ personal and congregate areas, which will help to compensate for cognitive, sensory and physical impairments.
  • Interviews the residents and families prior to admission. Coordinates the pre-admission screening and assessment process with Directors of Marketing and Resident Care.
  • Coordinates with the Resident Care Director, Memory Care Program Caregivers and families in the development of the 24-hour plan for the residents and updates the plan in a timely manner.
  • Coordinates delivery of services for each resident according to the 24-hour plan.
  • Attends Department head meetings and leads regularly scheduled Memory Care Program meetings.
  • Provides Caregivers with updates and in-service training programs and Memory Care programs.
  • Pursues educational development in Alzheimer’s disease and related disorders.
  • Assists the Marketing Director in promoting the Memory Care Program to doctors, hospital discharge planners and local community agencies.
  • Maintains recruiting of all Memory Care positions, conducting interviews, hires, onboarding, training, motivation, and retention.

  • Coordinates the Memory Care Program volunteer/mentor program.
  • Develops and maintains the community’s resource library.
  • Models our company philosophy, helping whenever needed to meet residents’ needs and working as a member of the overall team.
  • Participate as a Manager on Duty during assigned weekends.
  • Any other duties as assigned by the Executive Director and/or Leadership Team
Qualifications
  • Must be 21 years of age, or older
  • Must have satisfactory criminal background check in accordance with state requirements.
  • Must have satisfactory credit check.
  • Must read, write, speak, and understand English.
  • Must demonstrate patience, tact, enthusiasm, and positive attitude towards the elderly.
  • Bachelor’s degree in social work, psychology, gerontology, therapeutic recreation, or equivalent is desired. Previous successful experience in a memory care unit is required.
  • Registered Nurse/Licensed Nurse in good standing, meeting all applicable federal and state licensure.
  • Prior experience working with older adults with memory impairment.
  • Two years of experience in geriatric care.
  • Demonstrated supervisory experience.
  • Must have upper body strength adequate to bend, lift, shift, move, twist, and/or assist in moving articles of more than twenty-five pounds.
  • Must be able to squat, reach, and stretch without distress.
  • Must be able to tolerate extended periods of walking and standing.
  • Maintains professional conduct with associates. Supervisory experience is desirable.
