Job summary

The Springs Living, LLC is seeking a Memory Care Coordinator to lead the health services team and ensure high-quality care for Memory Care residents. You will collaborate with the community nurse and health services administrator to maintain accurate care details and guide staff in best practices.

The schedule is Tuesday–Saturday, 10pm–6am, with salary-based pay. You will supervise caregivers and med techs, participate in care planning, and support families with compassionate leadership.

Qualifications

  • Minimum 18 years of age.
  • Experience as a Medication Technician in a long-term care setting.
  • Experience creating comprehensive care plans tailored to individual needs preferred.
  • Ability to present information to groups/managers/residents clearly.
  • Ability to interpret and communicate instructions in written, oral, electronic, or schedule form.
  • Ability to read, analyze and interpret charting, orders or regulations.
  • Ability to write reports, business correspondence, and manuals.
  • Proficient PC skills including email, record keeping, databases, word processing, spreadsheets.

Responsibilities

As a MEMORY CARE COORDINATOR you’ll play a key role in ensuring our Memory Care residents receive the highest quality care while supporting families and leading our health services team of caregivers and med techs. In this position, you will work closely with the community nurse and health services administrator to ensure the accuracy and excellence of care for our treasured residents.

Schedule: Tuesday - Saturday, NOC SHIFT 10pm-6am

Pay: SALARY BASED

Responsibilities
  • Serve as the main point of contact for residents and families regarding care needs.
  • Manage resident charts and care updates, ensuring accuracy and clear communication with the team.
  • Lead care conferences with residents and families to ensure care plans align with their needs and preferences.
  • Manage caregiver and med tech schedules, finding coverage for open shifts.
  • Be in the rotation for the Staffing Phone
  • Be on-call med-tech shifts.
  • Assist in conducting continuing education and training for employees.
  • Assist in counseling and coaching employees as needed.

Qualifications
  • At least 18 years of age
  • Experience as a Medication Technician in a long-term care setting

  • Ability to effectively present information and respond to questions from groups or managers, residents, employees, and the general public
  • Ability to interpret and communicate a variety of instructions furnished in written, oral, electronic, or schedule form
  • Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Proficient PC skills including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.
