Job summary

Ellore Senior Living, operated by Oakmont Management Group, seeks a Memory Care - Care Provider to deliver daily personal care and support to residents in a warm, safety-conscious environment in Santa Clara, California. The role emphasizes preserving independence and dignity while following established care plans.

Responsibilities include assisting with ADLs, responding quickly to resident needs, and coordinating with supervisors and teams to maintain high-quality care standards.

Qualifications

• Experience preferred but not required; training provided.
• Must be at least 18 years old.
• High School Diploma or GED required.
• Excellent customer service skills.
• Strong desire to work with the elderly and care for their needs.
• Written and verbal communication skills with staff, physicians, residents and families.
• Background check and health screening including TB test; vaccination may be required.

Responsibilities

• Respond promptly to resident needs while maintaining dignity and safety.
• Report changes in resident condition to supervisor immediately.

Shifts, Time, and Days: Full-Time (AM/PM/NOC)

Pay Range: $20-21/hr

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications

• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
