The Oars Senior Living is seeking a dedicated Care Manager to provide quality care and meaningful experiences for residents with health ailments, dementia, or aging-related needs. This role reports to the department director overseeing Assisted Living or Memory Care.

Key duties include building trusting relationships with residents and families, maintaining dignified care, and collaborating with interdisciplinary teams to ensure high standards across memory care and assisted living neighborhoods.

Qualifications

  • Must be at least 18 years of age.
  • High School Diploma or equivalent required.
  • CNA certification helpful, but not required.
  • Ability to assist residents with activities of daily living as per plan of care.
  • Strong communication and teamwork abilities.
  • Ability to prioritize and multitask during shifts.
  • Experience with dementia/aging care preferred.

Description

The Care Manager's primary purpose and function is to provide quality care and experiences for our residents who live with health ailments and/or cognitive impairment resulting from the aging process, Alzheimer's disease, dementia and other illnesses. Our Care Managers report directly to the Department's Director (Assisted Living Director or Memory Care Director).

ESSENTIAL FUNCTIONS
  • Develop a naturally "inspired" relationship with our residents and their family members.
  • Ensure care delivery is dignified and of quality standards.
  • Understanding and respecting each resident as an individual with specific preferences, needs and values.
  • Have the ability to communicate with residents using compassion, empathy and validation.
  • Assist residents with activities of daily living according to their individual needs reflected on their plan of care. This will include, but is not limited to providing assistance with mobility, transfers, bathing, grooming, toileting, dressing, dining and activity engagement.
  • Active involvement in the day to day interdisciplinary operations of the Memory Care and/or Assisted Living Neighborhood(s) to ensure quality care and services are delivered. This will include completing neighborhood specific assignments relating to Housekeeping standards, Maintenance standards, Dining practices and standards, Wellness support and standards, "Programming That Cares" standards and practices and Community Relations practices and standards.

Daily light housekeeping duties which includes bed making, keeping room neat and tidy, and emptying trash.

Maintain a positive, inspired and engaged environment for all (residents, family members, team members and other professionals) visiting the community.

Responsible to report any change in resident condition/unusual incident to the Department Director, Healthcare Director and/or Executive Director in a timely manner.

Ability to communicate and/or report related challenges which may directly/indirectly impact delivery of quality care and services to the appropriate interdisciplinary Department Director or Executive Director.

Follows and practices all community and department rules, policies and processes set forth by The Oars Senior Living.

Engages in positive processes and practices; Comply with all federal, state and local laws and regulations as it relates to resident care and services.

Maintain all mandatory training requirements set forth by The Oars Senior Living and Community Care Licensing.

Requirements
  • Must be at least 18 years of age.
  • High School Diploma or equivalent required.
  • CNA certification helpful, but not required.
  • To provide the necessary care, supervision and services for a designated group of residents during your shift is required.
  • Be able to complete designated shift assignments accordingly.
  • General knowledge and understanding of the aging process, Alzheimer's/dementia care and basic ADL care experience preferred.
  • Knowledge and experience with managing behaviors associated with Alzheimer's disease, dementia and other related disorders which cause memory impairment is preferred.
  • Knowledge and use of proper body mechanics for back safety when assisting residents with mobility, transfers, turning and positioning required; Experience with mechanical lifts helpful.
  • Able to prioritize and multitask throughout the shift to ensure all duties are completed in a timely manner.
  • Able to work with a team, problem solve, and possess communication skills to effectively carry out all day to day job responsibilities.
