Job summary

PIH Health Medication Management is seeking a Nurse Practitioner or Physician Assistant to join our team in Cerritos, CA. You will provide specialized chronic disease care in collaboration with primary care providers and specialists, performing comprehensive examinations, preventive care, and ongoing disease management within the Medication Management clinic.

The role emphasizes independent patient care, with a focus on evidence-based treatment plans and coordination across PIH Health Group

Qualifications

• Current California Nurse Practitioner license or Physician Assistant license
• Current Basic Life Support (BLS) certification
• Graduate from an accredited Nurse Practitioner program or PA program
• Evidence of continuing education to support licensure
• Board certification by NCLEX/CBNS for NP/PA or equivalent credential

Responsibilities

• Provide care for patients with acute and chronic conditions in collaboration with primary care providers and specialists
• Perform and document comprehensive physical examinations
• Administer preventive care and conduct chronic illness surveillance
• Assess, diagnose, and manage health-illness needs within the Medication Management clinic

Position Summary

PIH Health Medication Management is a specialized subspecialty focused on comprehensive chronic disease management, working in collaboration with primary care providers and specialists across PIH Health Group and affiliated practices. The Nurse Practitioner or Physician Assistant works independently to provide care for patients with acute and complex chronic conditions. This role includes performing and documenting comprehensive physical examinations, administering preventive care, conducting chronic illness surveillance, and assessing, diagnosing, and managing health-illness needs specific to conditions supported by the Medication Management clinic.

Required Experience

• Current California Nurse Practitioner license
• Current Basic Life Support Certification
• Graduate from an accredited Nurse Practitioner
• Evidence of continuing education and self-development to support license requirements
• Board Certification: Board certified by the National Certification Commission on Certification of Physician Assistants/Nurse Practitioners.

Compensation Range:130K-150K
