Job summary
CHI St. Vincent in Arkansas is seeking a Certified Medication History Technician to gather and document patients’ medication histories, supporting medication safety and comprehensive patient care.
You will interview patients or caregivers, review current and home medications, and enter histories into the EHR in collaboration with clinical teams, ensuring accuracy and proper documentation.
A sign-on bonus of up to $5,000 is offered for eligible hires based on shift and eligibility.
Qualifications
- High School graduate or GED at hire.
- 3 years of Pharmacy technician experience.
- AR Pharmacy Technician license at hire.
- Certification: ECPT, PTCB, or Pharmacy Technician certification at hire.
- Must complete a written assessment.
Responsibilities
- Interview patients or caregivers to collect medication histories.
- Identify prescription, OTC, herbals, implants/devices, and illicit substances during history collection.
- Clarify incomplete information with dispensing pharmacy and relevant databases.
- Enter validated medication history into the EMR per procedures.
- Meet or exceed quality standards; consult pharmacists for unresolved issues.
- Identify and follow up on missed histories or follow-up needs.
