Job summary

Virginia Mason Franciscan Health seeks a Pharmacy Technician, Medication History, to collect and document patients’ medication histories, supporting medication safety and comprehensive patient care. You will interview patients or caregivers, review home meds, and record histories in the EHR while collaborating with clinical teams.

Qualifications

High School graduate or GED at hire.
At least 3 years of Pharmacy Technician experience.
Pharmacy Technician WA license required at hire.
Preferred: ASHP‑accredited Tech program or related experience.

Responsibilities

Collect and document allergies and current medication history.
Identify prescription, OTC, herbals, implants or devices, and any controlled substances.
Clarify incomplete data with dispensing pharmacy and provider using relevant databases.
Enter validated history into the EHR following established procedures.
Meet quality standards and consult pharmacists for unresolved issues.
Use software to identify missed histories and follow up as needed.
