Shift and Location

Shift: Monday - Friday (1300-2130)

Location: Singing River Gulfport Hospital

Responsibilities

The Medication History Technician (MHT) is responsible for accurately obtaining, documenting, and verifying a patient’s medication history upon admission, transfer, or discharge. The MHT works under the supervision of a licensed pharmacist and in collaboration with physicians, nurses, and other healthcare professionals to ensure medication accuracy and reduce adverse drug events.

Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.

Qualifications

• Education: High school graduate or equivalent required. Graduate of a pharmacy technician training program preferred.
• License: Must be registered as a Pharmacy Technician by the Mississippi State Board of Pharmacy.
• Certifications: Must possess a valid Pharmacy Technician Registration from the Mississippi Board of Pharmacy. Must possess or complete ASHP Medication History-Taking Certification within 90 days of hire/assignment. Must have de-escalation training completed by the end of position orientation (90 days); must have appropriate level of de-escalation training.
• Experience: Experience as a Pharmacy Technician preferred.

• Reports To: Pharmacy Director, Pharmacy Manager, Clinical Pharmacy Manager and Pharmacy Technician Supervisor
• Supervises: N/A
• Physical Demands: Work is moderately active: involves sitting with frequent requirements to move about the office, move about the facility, and to travel to another facility within the SRHS service area. Work involves exerting a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. Work involves using many physical motions in performing daily work activities; subject to exposure of body fluids, sputum and tissues, which may carry the hazard of infectious disease. Work involves using repetitive motions: substantial movements of the wrists, hands, and/or fingers while operating standard office equipment such as computer keyboard. Must be able to be active for extended periods and work schedules as assigned with potential changes for hospital needs.
• Mental Demands: Must demonstrate keen mental faculties/assessment and decision making abilities. Must demonstrate superior communication/speaking/enunciation skills, strong written and verbal communication skills, emotional stability conducive to high stress, and ability to work under pressure and meet deadlines. Attention to detail and the ability to multi-task in complex situations and maintain collaborative working relationships.
• Special Demands: Must possess superior customer service skills and professional etiquette. Must possess proficient computer skills (keyboard proficient) and familiarity with MS Outlook and Word, and other office technology.
