KDMC Ashland Hospital Corporation is seeking a Pharmacy History Specialist to obtain accurate home medication lists from patients at admission. You will assist pharmacy, nursing, and medical staff by conveying medication information verbally or electronically to care providers, ensuring complete histories across hospital settings.
Collaborate with multidisciplinary teams to support safe patient transitions and maintain compliance with procedures.
Responsible for obtaining accurate home medication lists from patients entering the hospital. Assists pharmacy, nursing, and medical staff by conveying medication information verbally or electronically to appropriate care providers. Ensures accuracy and completeness of medication histories in various hospital settings. Collaborates with multidisciplinary teams to support safe patient care transitions. Maintains compliance with organizational procedures and quality standards.
High school diploma or equivalent and vocational/technical training in a designated technical area.
0–2 years of experience in a healthcare, clinical setting, or a relevant area. An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part‑time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.
This position requires regular sitting at a computer workstation for extended periods; regular performance of tasks with repetitive motions (typing or data entry); intermittent standing or walking with objects weighing up to 10 pounds; occasional standing or walking with objects weighing up to 25 pounds; and occasional lifting, pushing, and/or pulling of objects weighing up to 50 pounds. Also involves occasional risk of back injury from moving, lifting, or positioning equipment or materials; occasional risk of injuries from use of equipment on the job; occasional exposure to hazardous chemicals and fumes including waste, cuts/punctures, bloodborne or airborne pathogens, and interaction with combative or violent people. Occasional job‑related travel may also be required.
