Corewell Health is seeking a Pharmacy Medication History Technician who will document and clarify patient medication histories to support safe inpatient care. The role involves interviewing patients or caregivers, interfacing with providers, and maintaining accurate profiles in the EMR.
The position requires a Michigan pharmacy technician license or intern license, with 1–2 years’ hospital/retail experience preferred.
In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution.
The Surgical Health Optimization Center was developed to help patients properly prepare for their upcoming surgery. Our goal is to improve each patient's health in order to decrease their risk of complications during and after surgery. A preoperative physical exam helps your surgical team determine if you are healthy enough to undergo anesthesia and surgery. This is sometimes referred to as being optimized for surgery. Knowing a patient's medications, doses, and their schedule of administration is essential to appropriately instruct patients and surgeons on how to best manage them in consideration of underlying medical conditions and the proposed surgery.
