Job summary

Proforma Health Inc. is seeking a Per Diem LNA/MNA to provide resident-centered care across independent living, assisted living, memory support, and nursing care in a Continuing Care Retirement Community (CCRC).

As a Per Diem LNA/MNA, you will assist with activities of daily living, safely administer medications, monitor residents, and collaborate with an interdisciplinary team to promote independence, wellness, and dignity for every resident.

Qualifications

• LNA or MNA credentials required.
• Strong patient-care and resident-focused approach.

Qualifications & Skills

• Licensed Nursing Assistant (LNA) or Medication Nursing Assistant (MNA) credentials

• Ability to administer medications safely and accurately
• Good observation, documentation, and communication skills
• Empathy, reliability, and teamwork in a community-oriented setting
