Job summary

AEIC Case Management LLC is seeking Texas DSHS-certified Community Health Workers (CHWs) as 1099 Independent Contractors for the Bridgewell program. You will provide resource navigation, care coordination, and Medicare-aligned support to diverse communities, including seniors and adults with chronic conditions.

Responsibilities include assessing needs, coordinating services, and documenting interactions, with up to 50% travel and flexible scheduling.

Qualifications

• Must hold valid Texas DSHS CHW Certification.
• Minimum 1 year experience as a CHW, case manager, or related health/social services role.
• Experience serving older adults, adults with chronic conditions, and low-income or vulnerable populations.
• Ability to navigate housing, transportation, food, and medical resources.

Responsibilities

• Conduct CHW-aligned assessments to identify social and medical needs for Medicare clients.
• Provide SDOH resource navigation (housing, food access, transportation, financial assistance).
• Assist clients in obtaining medical, behavioral health, and community-based services.
• Coordinate care and follow-up for clients with chronic illnesses or high-risk conditions.
• Document all client interactions and referrals in the electronic health record.
• Travel to clients as needed and maintain timely documentation.
