Job summary

Axis Community Health is seeking a part-time AWV Provider to conduct Medicare Annual Wellness Visits for Medicare patients in a multi-disciplinary outpatient setting. The role requires a medical degree (MD/DO/PA/NP), a California license, DEA license, and board eligibility or certification, with experience in primary care and EHR use.

Ability to work 16 hours per week, with flexibility to cover clinic needs, and bilingual English/Spanish is highly desirable.

Qualifications

• MD/DO/PA/NP degree from an accredited university.
• Minimum two years in a primary care clinical setting.
• CA Medical License in good standing and DEA.
• Board certification or eligibility with a set date to take the board exam.

Responsibilities

• Provide direct medical ambulatory services including examination, diagnosis, treatment, referral and follow-up in primary care.
• Collaborate with supervising physician for APPs and refer to higher level of care when needed.
• Maintain complete, accurate medical records in Axis policy timelines and use EHR effectively.
• Attend meetings, complete trainings including compliance and cybersecurity, and participate in care team activities.
