Job summary

Alpha Home Health and Hospice is seeking an experienced registered nurse to provide individualized, goal-directed nursing care to medical-surgical patients in home health settings. You will assess patient needs, plan and evaluate care, and educate patients and families to promote optimal health outcomes.

In collaboration with the healthcare team, you will coordinate care, apply evidence-based practices, and use clinical judgment to ensure safety, quality, and continuity of care in home
