Job summary

CINQCARE in Charleston, WV is seeking a Medical Social Worker to address complex Social Determinants of Health for Grace at Home family members. You will assess social risk, develop intervention plans, and connect families to healthcare payor and community resources.

You will build and maintain a comprehensive resource database, collaborate with interdisciplinary care teams, and support access, care coordination, and outcomes for individuals with complex social needs.

Qualifications

• MSW is required from an accredited program.
• 3+ years of social work in healthcare, care management, or community settings.
• Experience with interdisciplinary care teams preferred.

Responsibilities

• Direct social work services to family members with complex SDOH needs.
• Conduct psychosocial assessments addressing housing, food insecurity, transportation, and finances.
• Develop individualized care and resource plans with families and clinical teams.
• Provide ongoing follow-up, advocacy, and care coordination.
