Job summary

ICFAI is seeking a Medical Social Worker to assist patients and families facing social, economic, or environmental challenges related to illness. You will collaborate with the interdisciplinary team and community agencies to address discharge planning and follow-up needs across the continuum of care.

This role requires understanding of social work principles and the ability to access patient information and coordinate services for smooth transitions, ensuring patient- and family-centered care

Qualifications

• Bachelor's degree in Social Work required.
• Master’s in Social Work or related field preferred.
• License or certification in Social Work preferred.
• At least one year of hospital social work experience preferred.
• Understanding of principles and practice of social work.
• Working knowledge of community social and health agencies.
• Knowledge of human behavior and personality dynamics.

Essential Physical Requirements

• Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time.
• Must be able to lift and carry up to 30 pounds.
