Job summary

The Carpenter Health Network is seeking a PRN Licensed Medical Social Worker in Escambia/Santa Rosa County, FL. This role involves conducting patient assessments, managing casework for terminally ill patients, and communicating effectively with families and co-workers.

Qualifications

• Minimum of 1-2 years as a Licensed Medical Social Worker.
• Previous Home Health or Hospice experience preferred.
• Valid driver license, proof of insurance and reliable transportation for in home visits.

Responsibilities

• Conduct patient assessments and ongoing care of hospice patients.
• Communicate with patients, families and co-workers.
• Manage casework needs of terminal ill patients and family.
