Job summary

River Valley Home Health & Hospice is seeking a Licensed Social Worker (LSW) to bridge patients, families, and resources. You will assess psychosocial needs, provide counseling, and collaborate with an interdisciplinary team to ensure holistic, dignified care.

You will conduct comprehensive assessments, develop care plans, support patients and families, and maintain clinical records. This role requires a CSWE-accredited MSW, CPR certification, and a valid driver’s license for community visits.

Qualifications

• Master’s degree in Social Work from a CSWE-accredited program.
• Minimum 1 year in healthcare; hospice experience strongly preferred.
• Current CPR certification required.
• Valid driver’s license, auto insurance, and reliable vehicle for community visits.

Responsibilities

• Comprehensive Assessments: Evaluate psychosocial status of patients and families related to terminal illness and living environment.
• Care Planning: Develop and implement interventions based on thorough evaluations.
• Counseling & Support: Guide patients and caregivers through stress, coping challenges, and end of life issues.
• Documentation: Maintain accurate, timely clinical records for all referred patients.
• Resource Navigation: Provide referrals and serve as a liaison with community agencies.
• Team Collaboration: Participate in interdisciplinary care planning and regular team meetings.
• Advocacy: Help physicians and team members understand social and emotional factors impacting care.
• Quality Improvement: Actively contribute to performance improvement initiatives.
• Flexibility: Perform additional duties as delegated by the Clinical Supervisor.

Requirements
