Qualifications

• Master's degree in Social Work is required and work experience may not substitute for education.
• Currently licensed or eligible to be licensed as a LMSW by the Alabama State Board of Social Work Examiners.

Responsibilities

• Assist patients and families in accessing national, state, institutional, community and pharmaceutical resources.
• Provide social evaluation and counseling to patients, families, and groups and document in medical records.
• Participate in staffing, clinics, and rounds to support understanding of psychosocial situations.
• Facilitate discharge planning and arrange home health, hospice, equipment, or SNF placement.
• Coordinate referrals, MAP/PA paperwork, insurance and medication access, and follow-up to promote treatment initiation and SVR.
