Job summary

Endeavor Health is seeking a Medical Social Worker for the ETHS SBHC in Evanston, IL. The role is full-time (32 hours/week) with hours Monday–Friday 8am–4:30pm, no weekends. The Social Worker will support continuum of care, advocate for patients, and work within an interdisciplinary team.

Requirements include a Master’s in Social Work and an Illinois LSW or preferred LCSW. Experience with adolescents is required; 1–2 years in acute care is preferred.

Qualifications

• Masters degree in Social Work is required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensed in Illinois is preferred.
• Experience with adolescents is required.
• 1–2 years of social work experience in an acute care setting preferred.
• Spanish speaking preferred.

Responsibilities

• Develop and implement program policies and procedures.
• Coordinate social work services and referrals between the program and other agencies.
• Conduct individual assessments and short-term treatment for emotional problems.
• Provide crisis intervention and group treatment for psychosocial issues.
• Educate teachers, students, and guardians on mental health topics.
• Collaborate with school staff, sponsoring agency, and community partners.

Hourly Pay Range:

$31.06 - $46.59 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Location: Evanston Township Highschool Health Center - 1600 Dodge Avenue, Evanston, IL 60201


• Full Time | 32 hours/week

A Brief Overview:

• The Medical Social Worker will function as an integral member of a collaborative and interdisciplinary team, using assessment, implementation, coordination, monitoring, and evaluation skills. Supports Nursing Services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients, and in maintaining the quality-of-service delivery. The Social Worker of the Cross Care Continuum will also serve as a patient advocate.

What you will do:

• Participate with other staff members in developing and implementing program policies and procedures.


• Coordinate social work services and patient referrals between the program and other agencies providing social and educational services.


• Individual assessment, short term treatment, and referral of emotional problems.


• Crises intervention for emotional and situational problems.


• Group treatment for psychosocial problems.


• Family assessment, treatment, and referral for emotional problems


• Ongoing assessment of mental health needs in student population.


• Education of teachers, students, and guardian(s) and caregivers on topics related to mental health.


• Ability to work well in a team setting.


• Ability to implement philosophy and objectives of program.


• Ability to work cooperatively with sponsoring agency, school staff, students, and guardian(s) and caregivers.


• Ability to function effectively with other staff members as a team.


• Ability to teach classes and lead groups within the school setting.


• Ability to initiate relationships within the community and maintain liaisons with the lay and professional community.


• Ability to do public speaking relating to Health Center services and adolescent health.

What you will need:

• Education: Masters Degree in Social Work, required.


• License: Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR), required. or Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR), preferred.


• Experience: Must be experienced working with adolescents, required.


• 1 to 2 years of social work experience in an acute care setting preferred. Spanish speaking preferred.


• Unique or Preferred Skills: Able to demonstrate cultural humility and understanding of adolescent social development stages.


• Able to communicate effectively and work collaboratively with a range of stakeholders and team members.


• Able to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multidisciplinary team, and community agencies to achieve desired outcomes.


• Act as a subject matter expert.


• Teamwork-active participation with performance improvement and other approaches.


• Collaborates with the team members to develop effective work processes that my lead too improvement of work.
