Qualifications

• Master's Degree in Social Work from an accredited program.
• Licensed Social Worker in the state of residence.
• CPR certification on file.
• Access to dependable transportation.
• At least 1 year of health care social work experience.

Responsibilities

• Assist patients and families to understand social and emotional factors affecting health.
• Develop a comprehensive Plan of Care for patients daily.
• Instruct the health care team on community resources; act as Bereavement Coordinator when needed.
