Palmshomecare is seeking a compassionate Medical Social Worker to support home health patients and their families addressing psychosocial, emotional, and resource needs. You will assess patient needs, assist with discharge planning, provide counseling, and connect families with resources.

The role emphasizes coordination with the interdisciplinary team, documentation per Medicare/home health standards, and a strong knowledge of community resources.

Qualifications

• MSW required with active Florida LMSW/LCSW preferred.
• Florida license required or in progress.
• Home health experience preferred.
• Strong knowledge of community resources.
• Excellent communication and organizational skills.
