Job summary

AHS - Sherman Medical Center seeks a Master’s level Social Worker to provide comprehensive care in our hospital setting for patients of all ages. You will assess social, psychological, cultural, environmental, and financial needs and coordinate services to support discharge planning.

Collaborate with patients, families, the healthcare team, and community resources to develop and implement tailored care plans, ensuring high-quality support throughout the hospital stay.

Qualifications

• Master’s degree required for professional practice.
• BLS certification required upon hire and maintained.
• Licensure/certification preferred for advancement.

Responsibilities

Collaborate with patient families, healthcare team and communities to develop and implement plans to address identifiable needs.

EDUCATION, EXPERIENCE, TRAINING

• 1. Master’s Degree in a human services field (Psychology, Social Work, Counseling, etc…)
• 2. BLS upon hire and maintain current.
• 3. Professional Licensure/Certification preferred.
