Amedisys is seeking a Medical Social Worker to support patients and families in a per diem, weekday capacity. The role focuses on addressing health-related social, financial, and emotional concerns and delivering care per the established plan.

Responsibilities include coordinating with the healthcare team, educating patients and families on social work interventions, and managing transfers and discharges. Requires a CSWE-accredited MSW/PhD and CPR certification, with at least one year in

• One year of social work experience in a healthcare setting.
• Meets state and federal requirements for this position.

Responsabilités

• Assist patient and family with health-related financial, social and emotional concerns and coping.skills.
• Provide social work services per the plan of care.
• Communicate changes in patient status to healthcare team and care center staff in a timely manner.
• Educate patient and family and other team members on social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks to patient health and coordinate early interventions with the team.
• Other duties as assigned.

Pay Range: $40.00/hr to $55.00/hr

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
