Job summary

INTEGRIS Health Edmond Hospital in Edmond, OK is seeking a day shift, full-time Medical Social Worker to join our care team. You will conduct psychosocial assessments, assist with abuse/neglect referrals, and help develop discharge plans with the care team. Qualifications

• Must have MSW from an accredited school.

Responsibilities

• Performs thorough psychosocial assessments of patients and families.
• Provides assistance with evaluation/referral in abuse/neglect cases.
• Implements social service interventions based on assessments.
• Counsels patients/family in adjustment to illness and treatment.
• Assists with advanced directives and durable power of attorney.
• Collaborates with Case Managers on discharge planning.
• Liaises with physicians, families, and external agencies for referrals.
• Complies with required physical demands and risk management guidelines.

Responsibilities

• Conducts thorough psychosocial assessment of patient (including emotional, cultural and financial needs) utilizing all available resources
• Provides assistance with evaluation and referral in situations of possible abuse/neglect for adult and pediatric patients
• Implements social service interventions based on psychosocial assessment.
• Counsuls' patient/family in adjustment to illness, treatment and the process of grieving/conflict resolution
• Assists patients/family members in obtaining advanced directives and/or durable power of attorney for health care
• Works collaboratively with Case Managers and the health care team in formulating and facilitating patient hospital discharge plans
• Acts as a liaison between patients, family members, physicians, interdisciplinary team and external agencies and organizations making referrals as appropriate. Facilitate meetings for discussion of plan of care and discharge planning.
