Job summary

nshs seeks a Medical Social Worker in Chicago, IL, full-time (40 hours/week) with typical hours Monday–Friday, 8:30 a.m. to 5:00 p.m., and rotating weekends plus winter/summer holidays coverage.

Qualifications

Responsibilities

• Collaborates with Continuum Care Managers, Case Managers, Care Navigators, Physicians, Nursing, PCP and multidisciplinary teams to support patients with chronic illness and co-morbidities.
• Adheres to care management standards, patient-focused interventions and cost-effective practices.
• Coordinates across departments, clinics and community resources to ensure safety, transitions of care and overall cost management.
• Uses social work methods for assessment, education, collaboration and coordination of healthcare and community resources to improve outcomes.
• Provides support services to Endeavor patients and value-based contract members, including care coordination and counseling.
• Handles interventions in child abuse/neglect, domestic violence, guardianship, mental health placement, substance abuse, and advocacy at the patient level.
• Ensures safe care within policies, budgets, time management and productivity constraints.
• Initiates and facilitates care processes to improve workflow and patient outcomes.

A Brief Overview

The Medical Social Worker specializing in the Cross Care Continuum plays a pivotal role in providing comprehensive social work services across various healthcare settings and patient populations. This position requires a deep understanding of the healthcare system, strong interpersonal skills, and the ability to navigate complex medical and social situations. The Medical Social Worker will function as an integral member of a collaborative and interdisciplinary team, using assessment, implementation, coordination, monitoring, and evaluation skills. Supports Nursing Services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients, and in maintaining the quality of service delivery. The Social Worker of the Cross Care Continuum will also serve as a patient advocate

What you will do

• Adheres to established care management standards that incorporate evidence based protocols and cost effective, patient focused, targeted interventions.
• Provides coordination between multiple departments, multi-disciplinary team, medical clinics, and community outreach to gain knowledge of patient, assure patient safety, assure smooth transitions of care, and manage utilization and the total cost of care.
• Applies social work methods to skilled assessment, education, collaboration, and coordination of healthcare and community resources to achieve the following: assist patients to gain self-efficacy/management skills, achieve optimal functional health status, and improve quality of life. Assists patient/families, care team, and systems to achieve high quality outcomes through face to face and telephonic interactions.
• Provides support services to Endeavor patients and all of its value-based contract members including but not limited to care coordination and counseling.
• Provides intervention in child abuse/neglect, domestic violence, guardianship (temporary/ permanent), mental health placement, substance abuse, advance directives, adult/elderly/disabled abuse and neglect, and sexual assault.

What you will need

Master's Degree in Social Work required. Illinois licensure as LSW; 1 to 2 years of social work experience in an acute care setting preferred.

Minimum Education Level
